If I signed the application end of this month, the "period to be assessed" would look something like this:

From (YYYY/MM/DD): 2017/6/29
To. (YYYY/MM/DD): 2022/06/30

The answer to this particular question about the current (new as of June 2022) PR card application form is simple, as the straightforward instruction states:

Period to be assessed:

The date in the “From” field should be the date from the past five (5) years preceding this application or since becoming a permanent resident if less than five (5) years ago.

The date in the “To” field, should be the date when you sign and submit your application form.
So yeah, as @steaky incompletely suggested, for someone who has been a PR for more than five years, it should look "something" like that . . .
From: Year/month/day five years ago to the day as of the date the application is signed
To: Year/month/day being the date the application is signed and submitted

However, if the applicant has not yet been a PR for five years, as of the date the application is signed, it should look like:
From: Year/month/day for the date the PR became a PR (date of landing)
To: Year/month/day being the date the application is signed and submitted

NOTE:

This is a new form. It is always important to read and follow the instructions, which includes the "Guide" for the respective application. But it is especially important for any new forms, such as this. Moreover, not only is this a new form, it is quite different from previous PR card application forms, which further elevates the importance of reading, carefully, and following the instructions. And, actually, the content of the form actually varies depending on how various questions are answered. Note, for example and in particular, the same pdf form is now used to make a PR Travel Document application, and the particular content of the application being completed will be different if in response to question 1.1 the applicant checks the appropriate boxes for making a PR card application or those for making a PR TD application.

I intend to start a new, separate thread to discuss the new forms and new instructions and new guide. The latter are, apparently, still in the process of being updated for the new forms.

It might be prudent, for those who can, to wait to make their application for at least a few more days to be sure any wrinkles in the forms, instructions, or guide have been ironed out. Also note, and this alone may be a wrinkle, the form's instructions are not in the form itself, so there are separate webpages for the instructions and the guide.

See instructions here: https://www.canada.ca/en/immigration-refugees-citizenship/services/application/application-forms-guides/imm5444.html
See guide here: https://www.canada.ca/en/immigration-refugees-citizenship/services/application/application-forms-guides/guide-5529-applying-permanent-resident-travel-document.html

The trick is that the webpage for downloading the form has an expandable section for showing the instructions. USE THIS. When in doubt, follow the instructions; otherwise, yep, follow the instructions.

What incompletely suggested???? Mind your language. @dpenabill

OP didn't say whether "has been a PR for more than five years" nor "has not yet been a PR for five years". I merely shared what I'm going to do in this situation. You on the other hand, really needs a magnifying glass.

Criticism noted.

It also warrants noting with some emphasis no one was asking what YOU would do. And who cares? Who knows to what extent YOUR situation is at all relevant to the question being asked? The OP was asking for guidance on how the OP should complete the application.

But OP didn't specified what situation. Therefore, I shared what I would do if I am in similar situation. Btw, you can intend to start a new, separate thread to discuss the new forms and new instructions and new guide. But again who cares about what you intend to do if it's relevant to the question being asked. The OP was asking for guidance on how the OP should complete the application (not what your intention was).

Hi,
I have applied my PR Card renewal through PR Card portal.
Application was received on 21st Sep 2021. I even did not get any email with Confirmation #. I called IRCC and they gave me Confirmation #.
I am still waiting as of today to receive my PR Card. I have my flight Next friday and travelling to india and getting worried. I have called IRCC and agent said they cant do anything and advised me to apply for PRTD before travelling to India. also, i am not able to link my Pr card portal application to online IRCC account. when i called IRCC, they only said Application is in progress. its been 4 months now.
is there any one who received their PR card and applied during Sep 2021 timeframe?
