The answer to this particular question about the current (new as of June 2022) PR card application form is simple, as the straightforward instruction states:

From: Year/month/day five years ago to the day as of the date the application is signed
To: Year/month/day being the date the application is signed and submitted
However, if the applicant has not yet been a PR for five years, as of the date the application is signed, it should look like:
From: Year/month/day for the date the PR became a PR (date of landing)
To: Year/month/day being the date the application is signed and submitted
NOTE:
This is a new form. It is always important to read and follow the instructions, which includes the "Guide" for the respective application. But it is especially important for any new forms, such as this. Moreover, not only is this a new form, it is quite different from previous PR card application forms, which further elevates the importance of reading, carefully, and following the instructions. And, actually, the content of the form actually varies depending on how various questions are answered. Note, for example and in particular, the same pdf form is now used to make a PR Travel Document application, and the particular content of the application being completed will be different if in response to question 1.1 the applicant checks the appropriate boxes for making a PR card application or those for making a PR TD application.
I intend to start a new, separate thread to discuss the new forms and new instructions and new guide. The latter are, apparently, still in the process of being updated for the new forms.
It might be prudent, for those who can, to wait to make their application for at least a few more days to be sure any wrinkles in the forms, instructions, or guide have been ironed out. Also note, and this alone may be a wrinkle, the form's instructions are not in the form itself, so there are separate webpages for the instructions and the guide.

The trick is that the webpage for downloading the form has an expandable section for showing the instructions. USE THIS.
When in doubt, follow the instructions; otherwise, yep, follow the instructions.
