POST MEDICAL APPLICANTS - Waiting for VISA

Hi,
I got Medical Request today at 2.59 PM
I specially thank to Ashu Bhai, Parth and all guys who helped me.

But, I received email as below

You have a new message(s) in your account.
To view your message, sign in to your account.

After signing into my account ,I got 4 same messages (1 for Principal Applicant & for 3 Dependants) as below

Medical Report - Section A Client identification and summary (IMM 1017E) - September 6, 2017 05:28:37 a.m. EDT

I have some questions for Ashubhai,

1. What is this Medical Report form (IMM1017E)? whether, this is the just client Identification form ,I have to carry with me for Medical Test,Please clarify.
2. I never received any messages for paying the PR fee & Bio pages of passport,eventhough I have to pay & submit these things with my medical report,Please clarify.
3.Whether, I have to carry any photos for Medical Test?
4.What are the preparations I have to do for my Medical Test?

Answer to your queries:

1. This is the form which you need to take a print and give it to hospital. They will do the rest. Take all members forms with you.
2. If you have already paid your RPRF and sent your Bio pages (if requested) then no need to take any action. They do not confirm the receipts for the payments you do. They just will tell you Medical received and Medical Passed or Failed.
3. Yes, normal passport size photos (2-3 per person). Check with the hospital while you book an appointment with them.
4. Nothing. Just make sure you have 4-6 hours time and bring your child's all vaccination reports and if anyone of your family member (who is coming with you including you) have any medical conditions then those reports and other documents.
