I specially thank to Ashu Bhai, Parth and all guys who helped me.

But, I received email as below

To view your message, sign in to your account.

After signing into my account ,I got 4 same messages (1 for Principal Applicant & for 3 Dependants) as below

Medical Report - Section A Client identification and summary (IMM 1017E) - September 6, 2017 05:28:37 a.m. EDT

I have some questions for Ashubhai,

1. What is this Medical Report form (IMM1017E)? whether, this is the just client Identification form ,I have to carry with me for Medical Test,Please clarify.
2. I never received any messages for paying the PR fee & Bio pages of passport,eventhough I have to pay & submit these things with my medical report,Please clarify.
3.Whether, I have to carry any photos for Medical Test?
4.What are the preparations I have to do for my Medical Test?
