Any idea about the format of this 'one and the same person affidavit'? I found few templates online, not sure if it is sufficient. More importantly, who should sign it? is it printed on simple paper? etc

Hi if it is only initials that are not expanded in some documents you do b not need to submit anything. I Had initials not expanded in some documents and got pr without any issue. If you notice ircc never asks for one and same have affidavit because if it looks like the difference can be explained with affidavit then they will accept without affidavit. All the best!

I wrote a simple affidavit for one and the same explaining my name variations in my certificate Got that signed by notary in UPS store. Attach this document in the place holder

Its option 6. My wife faced this same issue. She chose option 6 and submitted a one and the same person affidavit which mentioned all her different names as it appeared in Passport, Educational Documents and WES ECA... This should do the job.. we faced no issues in our application..
HI I am in the same situation. I have everywhere single name but in one of my past employment it is with surname which was a mistake. Should I also chose option 6 because it is not a legal name change but a professional name On chosing option 6, does it give you space to explain about other factor

Hey guys! What if the order of my name is different on my education qualifications? On one certificate, it is written in the order of surname - given name and on the other one, it is written in the order of given name - surname. Do I really need to get an affidavit for just a change in the order as in my passport, there is no specific order and both surname and given name are mentioned in two separate fields?

I am in the similar situation. My name is consistent in all of my documents with XXXX surname and my father's name is also consistent in all of his documents but with YYYY surname (different last name). therefore, my passport has a special remark stating that person was previously know as YYYY surname. what name type should I select from 6 options provided while submitting the application. Obviously, option 2,3, and 5 are out. I am aware that I need to submit "same name affidavit."

1. Birth
2. Nickname/Alias
3. Name before marriage
4. Former Name
5. Son/Daughter of
6. Other


In reality, I don't have a former name but just a discrepancy between my father and my last name. Different last name are due to the fact that, my cast uses two types of last name and they both pertain to the same sub-cast. This situation is very common in India.
