Post Graduate Work permit problem due to passport expiration

Hi.
Due to my expiring passport, I only got 1.5 years of PGWP. (Was support to get 3 years since I was in 4 years program at a university)
I was aware that with renewed passport, I can redeem the remaining 1.5 years of PGWP.
SO I did get my passport renew, however,
I do not know how to apply to renew my post graduate work permit.
Should i apply for it like I did the first time as a student with a note stating my problem, old passport, new passport, and post grad work permit that is about to expire?
or is there another way?

Yes apply to CPC-V for your PGWP with all of this information as well as a well-written cover letter. Include all documentation and be very clear and sequential in your submission.

For this type of situation, in addition to the IM5710 do you need to submit the fee as well when extending the visa to it's full 3 years? do I include the original of my old permit or just a copy?
