Reply to the CSE raised is that they have forwarded our query to the concerned person and we may also contact the ADVO directly as our file has crossed the average processing time and ADVO only can explain the reason behind this delay.
After that raised CSE to ADVO and now waiting for their reply .

Actually I nominated in SINP program and regarding to the visa I don't know but as I am submitted my application from Saudi Arabia and according to CIC web site the visa office for PNP from Saudi will be ADVO

How long did it take you to get SINP nomination?
 
Dear everybody,

I thought I would just let you know - I have passed medical without MR.

It was taking too long and I was about to move out from Dubai, so it was just more convenient for me that way. Was a bit risky, but all worked out.

If you are interested, that's how I did it:
-found certified clinic and made an appointment for UPFRONT medical (as I did not have email from CIC)
-After medical, contacted ADVO via CSE with upfront medical UMI# scan attached (clinic will give that to you after you pass medical) asking to connect this UMI# with my application. Note: Have not received a response from ADVO, but they connected.
-later additional test was required. passed that test in different certified clinic already in USA.
-after additional test was passed, called initial clinic in Dubai, asking to submit my medicals.
-medicals were submitted on 2nd of Jan. on 11th of Jan the line "Medical results have been received" has appeared on my CIC.

Now waiting for them to start background check etc. and to receive passport request as the next communication from CIC.

Yeah, I would think so. The only time you don't want to do upfront medical is along with your application. Processing might take up to or longer than a year, like in our cases, and then you will have to pay again and retake it.
As we are already waiting for MR for 11 months, application should be finalized soon, so I thought I will give it a try.

Yeah 11 months of waiting is too long and it might take longer because there's a newly appointed Minister for IRCC.

We will be travelling in a month from now and don't know if we will try also upfront medical.
