No. I have a proper medical request document attached in the email.
your wife's name is in there?..coz the second email i receive was just the form blank name and other info..with only my address here in canada where in my husband is in the Philippines..
 
For all already got ME app, when you checkclient application status, is there alreadt showed like we sent you ME request on xxx,2017, the information like this? or you just got email....thanks
