PNP PR APPLICATION SEP 2014 APPLICANTS IMM5669 - CIC Query abt personal history
Hello,
I am a new member, and could not find a place where I can post the question about IMM 5669 - personal history, pardon my ignorance!
I am a PNP - PR applicant, CIC received my application on September 12, 2014. Meds done on Dec 16/2014.
On June 4th, 2015 I received 2 emails from CIC requesting for the following:
1. SCHEDULE A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Provide your detailed personal history from September 2004 to present date. Do not leave any sections blank, or gaps in time. You must write 'N/A' in any sections that are not applicable to your situation. Include all: jobs, periods of unemployment, study, vacations, or being a homemaker.
I had submitted my personal history in the following format for the last 10 years:
Sep 2004 - Apr 2007 - working - (designation) Sales Rep - City - Status -Employer Name
May 2007 - Aug - 2012- working - (designation) - Sales Rep - city - status - Employer Name
Sep 2012 - Aug 2013 - studying (course name) - city - status (student) - name of University
Sep 2013 - Sep 2014(present date at the time of submission) - working (designation) - Audit - city - status (worker) - Employer name
I have traveled to a few places for vacation while I was working during these 10 years, which I have detailed separately in the Supplementary Information (IMM 5662).
Can you let me know if I have been asked for Schedule A (IMM 5669) because I have missed out any information or is there a gap in my history? Or is it that CIC needs to know what I have been doing since Sep 2014?
2. BIRTH REGISTRATION/CERTIFICATE: Birth registration/certificate. Please note: Original documents must be received by mail. Notarized documents are not acceptable in lieu of original documents.
I had submitted a copy of my birth certificate (in regional language from India) along with a Domicile certificate in English (which mentions my date of birth, name of parents, and has a government seal), my grade 10 certificate in English which mention my DOB, and a letter explaining that I have submitted this since my birth certificate is in a regional language.
Since CIC has not accepted this, (I feel so dumb now!!) I want to know if I need to submit my ORIGINAL birth certificate in English?? or I need to submit a copy of my birth certificate duly translated in English? I would also like to know if there are any other alternatives to the birth certificate that I can arrange for from here? I am currently residing in Winnipeg. I have to resubmit all documents before a month - July 4th!!
