Yes that is how u do it. No need for PR or citezenship. That is just ridiculous whoever said that to you. It does require $5 and takes a month.

Actually if you reside in Canada and choose Privacy Act insted of Access to Information Act, you don't need to pay anything. I've done it twice that way already.

Mine is the same. Home address at left and mailing address at the right.

Is it completely safe? I mean no further information is needed to apply under Privacy Act? I am planning to apply for GCMS Notes today. Let's see. 9 months and still no AoR, I am freaking out right now.
