I am not sure but there might be a reason for that...my spouse is here on work permit but her work permit was issued last year so she still has valid medical...thats y they didn't ask for her medical....Thx.. wish same for u.

Yes you just need to go to the nearest border (e.g- US border), tell them you are doing landing for PR, you exit from one side and re-enter into Canada side, since you don't have US Visa. If you have US Visa, process is still the same, just if you want to spend time in states and then come back to Canadian side.Wait what if i am already in Canada?
