Hi there sset12..how did you fill up the generic application form for canada (IMMM0008) under application details 1 and 2?Was it 1-Economic then 2-Provincial Nominee or 1-Family then 2-other relatives?I got my MPNP nomination thru a family support of a cousin..thanks in advance :)

Hi grayhair,

did you get family support from one first cousin or two distant relatives ? either ways it does not matter in the form, you need to put economic and then provincial nominee, thats it. Cheers

Well, I check "spouse" when I filled out this form for my wife. You need two forms to fill out, one for you and one for your wife. When you do for yourself, you leave next line, spouse line blank, and only fill out you and your children, father.mother, sister... information. Then, you fill out for your wife, you need write your wife name on the line with "Spouse", not applicant because you are applicant not your wife. Then leave your entry blank, which is top line, and fill out your wife's mother, father, sister, your children, ... information. It is way I did, but you need to double check. another folk agreed your way to do, so I am not sure. But I think I am right.

All other folks here, could you help us which way is right to fill out for the spouse? Thanks.

no, if you are the principal applicant, then your name must be on the "principal" row and your wife's detail should be in the "spouse" row... that means yours and your wife's details on the first two row must be the same(except if you have different address) -Section A of IMM5406

I guess there is some confusion:

As per my understanding the Additional Family Information form has to be filled by both Principal Applicant and Spouse. My question was "When my Spouse fills out the form, her status in the "Relationship'' column becomes ''Applicant'' right?". In my case i am the Principal Applicant.

So I guess, I complete an IMM 5406 as applicant, and she completes another one as applicant.

You are right, I need to fix my form. But, you and your wife need to fill out forms separately. When your wife fills out form, she puts her name at top row as APPLICANT, and write your name as SPOUSE. This is way I am going to fix.

Do you think both my wife and I need to C0-sign each form, or I only sign mine and she signs hers? I think it is right , but maybe wrong. What do you think.
 
Sorry the confusion guys. When I was applying for SINP, they did asked for the copies of the federal forms.
As I did not know what to do I called SINP. And they said to me to do this way. Put the wife as a applicant on the second form and she signs.

we are talking about form IMM 5406, Section A, right?
your wife must fill out the second line as "Spouse" but she still needs to fill out the first line (do not leave it blank) and specify you as the applicant, therefore you have two forms, the first two line have the same info...

this is how i filled out the form for my husband...

Dear ALL,

When I fill out schedule A form online, I directly type information to PDF and I can save it. But after saving it, I cannot change contents at all. My question is: Space for some entry is large enough so I cannot type all information in that spot and part of will lost. Is there any way to fix it? I try to print out and use type writer but hard to get alignment, or I can use pen to complete the missing part, but not pretty. Is there ADOBE software can change the font size so I can make it?
