yes, thanks for the reminder. I already did, the minute I got the info from sking. actually I already filled up and saved that form because it was in the ADVO checklist but since we submit our file to CIO, so i did not include that. Good thing I saved all forms (per ADVO checklist) before the procedure changed into CIO.
Keeping my fingers crossed that my AOR is coming within this week or before May 31st.
