1. Should dependant children who belong to the sponsor and applicant, who are already Canadian citizens (by descent) be included like in: IMM008 question 3, IMM008 Dependants section and IMM5481 question 5; and do these children who are citizens need to get the medical done, and submit photos?

2. In question 4 of IMM5540 in the Mother information address box, there’s a box that says: ‘same as opposite or ‘. What does this refer to?
 
Not sure about your first question. For the second question, you check the "same opposite" box if your mother's address is the same as your father's, which it is next to on the page.
 
You do not include dependent children if they already have PR status or Citizenship... they are included on the application as additional family. so no medicals, no photos...
 
But it says exactly 'same, opposite or' and a box beside it. How does it make sense for an address to be same and opposite at the same time. Frankly I find the wording to be very confusing since these two words are antonyms. Right? If my mother is deceased, should i just write that in the box?

Areas that ask for the dates of a period of time, “From” and “To” boxes, if it is present circumstances, what do I write in the “To” box? (eg. An address that I currently live From 10-12-2010 to ?)
 
OH sorry and another question:


I do not have any documentation that has the exact date of my Canadian Citizenship, would the year suffice in question 7 of IMM1344?

After you print out the completed form, hand write in "present" or "current"
