Hi there,

I had some questions about filling the open work permit and PR application forms under the live-in caregiver program. I am a live-in caregiver who is want to apply PR and open work permit for my common-law too. (She is already in Canada as a visitor visa for more than 1 yrs and we have prove that we stay together.) We are trying to fill out the form "IMM5409E"

The questions are :

What is the "Commissioner of Oaths" ????? I'm not sure is it a lawyer or what ?
What should I do with this form ?????

We are trying to find the guideline but there is no guideline for this form. Can you guys please help us ? :-[

A Commissioner for Oaths is a Commissioner for Oaths. Just type this into google together with the city you live in and google will show you all Commissioners for Oaths in your area ;)

I never used this form for my common-law application so I can't help you with it
 
Like little_apple said, just google Commissioner for Oaths :) - per the Sponsorship Guide you also have a few other options:

Who can certify copies?

Persons authorized to certify copies include the following:

In Canada:

a commissioner of oaths (authority to certify varies by province and territory)
a notary public
a justice of the peace

Outside Canada:

a judge
a magistrate
a notary public
an officer of a court of justice
a commissioner authorized to administer oaths in the country in which the person is living

Family members may not certify copies of your documents.



We had CIC request a copy of this form along with our PPR last week (despite the fact that guide clearly states it's only needed if the partner is a co-signer argh!). Just fill it out with your details - don't sign at the bottom yet - and then both you and your partner need to bring it to a Commissioner for Oaths (or someone similar) where all three of you will sign and the C of O will usually stamp the form with his/her name, location, and registration #.

Easy :-)
