Do I need to photocopy and notarize all the pages of passport or only the information and the last page where foreign currency is endorsed? I have a cancelled previous USA visa stamp ( Cancelled Without Prejudice ) .

I think it depends if you are applying online or on paper... In my case, I applied on paper and the VAC Bogota only requests simple copies of all documents and in original language... So, are you applying online or on paper?

Only photocopy of first and last page of the passport. You must also disclose previous visa refusals, if any, on the main application.
