Personal Activities Section of eAPR

I got my ITA today and trying to fill out the eAPR. I have some questions on the 'Personal Activities' section.

"Provide the details of XXX's personal history in the last ten years (or since his/her 18th birthday if this was less than ten years ago)."

1. I worked with my previous employer for 4.5 years. I joined them in India and quit them in the US. I travelled extensively for work while employed with them. I lived and worked in 3 countries and six cities and visited another city on business during this period. Should I create an activity for each of this period?

2. Or Can I just state it as one activity with the details of the location where I lived/worked last with them?

IMO either way would work as long as you provide all the details. Probably easier if create separate entry for each country

Thanks for the suggestion @ContactFront. I was planning to keep it as one activity and provide the travel details in travel history section. That will probably keep it simple.
