Payment receipts

Hi everyone!

So I just payed me and my husbands fees and am about to fill out the receipt. Since it's a sponsorship application do I fill out my (principal applicant) or my husbands (sponsor) 's date of birth on the receipt? And also do I send two copies to CIC or just one?

You send only one copy.
Fill the receipt with any name. It's for refund if needed.
