You have to reach out to IRCC and inform them of this immediately. I'm totally shocked that since January you're only just finding out as well. You will have to get your documents re-issued. In these times, pray this is sorted out quickly. The good thing as you said; you are sure the error is from them and not you.

I already sent the necessary documents on Feb. 17, 2021 to Operations and Support Centre Ottawa, via DHL. I am yet to get any email or response from the office on the action being taken to effect correction. Several Web Form has been sent yet no response .
Which email address can i use to contact IRCC ?
I am deeply worried about this especially knowing that my COPR will expire on May 12th, 2021.
If I read this right you have done the needful and all you can do is wait. You might want to email Accra since they approved or requested for your passport ACCRAIMMIGRATION@international.gc.ca also find the CPC case office details on the CIC website. Call the contact centre as well if this will speed up things (i hope). You shouldn't be worried about something you can't use as well. Get your documents corrected then worry about it expiring

Could that be the reason why my Visa Details / information did not show up while it was scanned at the airport? What is the possible turnaround time to resolve such issues?

Hello all...my dependent’s passport got delivered to VFS Lagos on Feb 2 and still waiting for the passport to be returned. Anyone here submitted passport recently?, how long did it take to get it back?
I’ve tried to...couldn’t reach them. And no I haven’t paid the courier fee.
I meant that I sent the passport to them for visa processing and UPS confirmed they delivered it to the VFS on Feb 2. So I don’t know if the passport has been processed yet. I’m just trying to find out the time line of people that sent theirs recently and how long it took to get their stamped passport back.
Use the VFS tracking ID they sent to you. If you don’t have one get in touch with VFS or search your email thoroughly. People have had timelines of 1-2months in getting passports back.
