Passport submission package to India VFS

Hi Group, Request you please assist with the information.
As part of preparing the necessary documents for passport submission to VAC India, can you please confirm if any other docs in addition needed to the list below?

1. Passport original
2. Passport request letter sent from IRCC
3. VFS courier & SMS form ( request you to please share the template, so i can double check)
4. Consent form ( only for 18 and above)
