Hello all Forum Members,

From Local visa office - New- Delhi, My parents just recieved mail in India to send the following information as below:

I need urgent advice from you guys as CIC given me only 45 days to complete all documents including Medical.

Questions:

Where I will get medical forms? Nothing comes with mail in India. - Do i have to download medical forms from CIC website if yes please provide me the details please.

Where I can get Schedule – A forms?

CIC did not send any Medical forms with mail and CIC given me only 45 days to complete all the required documents.

I am little bit in worry about these two issues.

Mail is a below:

This is refer to your application for admission in Canada

Compete or provide proof of completion of the immigration Medical examination for yourself and your spouse

attached details of Foreigh Travel Forms for both of you

Please priovide schedule - A forms fo yourself and your spouse which are completely and accurately

original passport for both of you.

Your urgent help regarding this matter would be highly appreciated

When my folks recently went for Meds ( visit visa ), there was a medical from with their file #, this was the same form for Permanent Residents as well. This form mentioned things the doc has to do and once they finished the test, the doc filled this http://www.cic.gc.ca/english/pdf/kits/forms/IMM1017SCLE.pdf and gave a copy to them

Perhaps you can call the DMP and tell them you got this

In addition, what is this Foreign Travel form? did others get this as well. Didn't we fill this info in the additional docs

SINGHNX01 and VEERU should be next in like to receive medical requests from NDL. Their LVO timeline is quite close to that of Rick2007. Keep an eye on the mail friends!

I am 6th march 08 but no sign of even an AOR, lol

Its a country-specific form that needs to be filled if the applicant is applying from India:

No! form for parents is different..
it has application no ,UCL on top left..
on bottom there is written "IMM 1017 F"

they usually send within the mail..but dont worry doctors have that form too.
when we went to medicals the doctor asked us to get colored photocopies of those forms and we said we already have these..

regarding form A ,.its the one which sakamath posted..i compared it with mine..
you have to fill one per each family member

I'm going by the AOR date. I guess that's what would determine the queue once the file is transferred. I hope you get your AOR soon!

My father/mother have 4 passports each, traveled to 7-10 countries, visited etc for around 20-30 times?

Who the hell remembers all that, and forget immigration, even I can't figure out from the entry exit stamps

It says be truth ful etc, how do I figure it out

Hi Rick2007 - First of - congratulations on the progress we see on your application.

Regards Schedule A - is this the form CIC is looking for? http://www.cic.gc.ca/english/pdf/kits/forms/IMM5669E.PDF

Medical forms have Sections A..D (or something like that) not Schedules. Call up the nearest DMP and inform them that you have not received the forms from CIC - better still - call up CIC ( @ NDL) directly and inform them that the forms were not included in the letter. For medicals, you need original passport, so do not send the ppts until your parents have completed their medicals.
