congrats ,u applied in 2007 and got medical request today but i heard ppls who applied from sep 2007 to dec 2007 got medical request in month of september.

We are told to fill IMM008, Schedule 1 form and family member info forms.

Luck786 - we definitely have a special bonding and have held each others hand in turbulent times almost everyday. You assumption may be correct that after my parents have got visa, I'd stop visiting this forum. However, honestly speaking this forum has been my second home. I have been visiting this forum at least 25-30 times in a day. So, you can imagine how many times I'd visited this forum or how much I lived on this forum.. I may not have posted a post on my each visit but just visited for reading posts. It will not be easy for me to stop visiting this forum, as it has become part of my everyday life and runs in my veins now. I will be visiting this forum even after my parents have got visa. I think it would take months before this forum completely flows out of my blood.

Hi all (especially to those wondering about surveillance codes),

11 means you don't need any surveillance. My parents' Canadian officers clearly said that any code that has 2 in it means you need a medical surveillance and if the VISA officer did not provide you a document that explains your need of surveillance, the BORDER OFFICER will give you. It is your border officer's responsibility to further document, explain, and instruct you for your medical surveillance if you need it. This is SOP at POE for permanent resident as well as temporary resident. So if you did not get anything from visa and border officers which explains your surveillance code, that means you don't need any surveillance.

Its IMM5669 which you have to fill for each family member

It's actually Schedule A, and as gaggy said, IMM5669 - background declaration - something which we have already done
