After clicking on the link above we find a form with 25 pages. My query is that should I only fill the Supplimentary Information part in this form or Should I have to fill all pages of the form available on the above link?

That guide is a generic list that used to be required. If CHC only wants the supplementary forms, send only those indicated.
You can mention in the letter that if they want more info or forms then they can send an email.
Also provide CHC your email, so that they can communicate via email too, faster than mailing.
