On the passport request letter, they are asking for the information below. Does anyone know if there is a form for it? Do we just put it on a blank piece of paper and send it? Any input would be appreciated.
You just need to fill the consent form and send it with the passport.

Hi , i received a biometric request for my parents and they submitted the finger print but then we realize that the Date of Birth that was on my Mom's biometric letter was not correct ( In application i provided correct date ) Now since biometric is submitted i am not able to view Biometric Validity online for my mom unless i use the incorrect date as per letter. With correct date of birth the online tool cannot find the result for my mom.

Has any one come across such information?, i have submitted the webform and waiting . Do you know if we should get another biometric at our own cost ?

I have one more question about the police clearance , My parents lived in Canada more than 6 month , how to we apply police clearance here while they are in india ? How do they submit the fingerprint , I tried to check on RCMP website and i am still confused how to get the fingerprint .

For my parents in the medical request letter the country of birth was wrong but we let IRCC know by web form and then they ganerate new request letter. So my suggestion is to let them know correct date of birth by webform with explanation letter. They will correct it from their side.
Send them biometric proof that you did biometric.

You don't need PCC from Canada. My parents are in Canada since 2010 on the visitor visa. They went outside canada for few times only for vacation. We did not provide PCC for canada. See my signature for timeline.

No. You don’t need to get a police certificate for your time in Canada.

 The country where they currently live, and
 Any country they have resided for a period of six months or longer since the age of 18.

Did they mention specifically canada? Because those are the standard format for PCC request.

So just to clarify , even if they lived in Canada for about 8 months they do not need to submit police clearance of canada ? They only have to submit PPC of India ?

Yes because my parents are here since 2010 as a visitor and we did not provide them canada pcc. they just went outside of canada for few times for vacation only otherwise they were here parmenent in canada.

What consent form? For me, they asked for
1. Passports
2. Photos
3. Passport request letter

Is there a anything else they asked during PPR? Probably I didnt understood GusCan's question.
