If you know year or approximate year. Put that. Use 01/01 as month n date. Write an explanation letter. Refer to question 3 of sch a.

I noticed its a PO Box address, what courier company you used ? can we use any fedex so we can get signature confirmation ?

I used fedex, it was delivered with no problem (Although it was signed by a different person that most of the colleagues )

Do we need to certify or notarize photocopies of original supporting documents like marriage certificate, birth certificate? or we can just send photocopies with application.

No, if they are in English. Otherwise notorized copy of original document is attached with Original translation of the same document.
