Hi, need help with Form IMM 5406 Additional Family Information.

My dad is the principal applicant. Since it says that my mom need to complete their own copy - in her copy do I still put my dad as the Applicant? or do I switch the roles?

There is difference between Applicant and Principal Applicant. in IMM5406 Both mom and dad are applicant. In you mom's Form mom is applicant and in Dad's form Dad is applicant.

I need help regarding Surname column, my mother has not mentioned Surname in her Passport. Means Surname column is empty in passport. Now how to fill the Form because surname is required there.

My parents are currently in Canada on super visa since Feb. I asked IRCC on twitter and they said I should write country of residence as Canada if they are in Canada. So, In Schedule A Background information,
a) Question 6 (Personal History), Should I write visitor from Feb 2019 -ongoing and in second row, 2009-Jan 2019 - home country?
b) Question 10 (Addresses), Feb 2019-ongoing - my address in Canada and 2009-Jan 2019 - home country address?

Also, in IMM0008, question 10, should I write a tentative date in future when they plan to leave?

Chill! They will not return your application. If needed, they will ask the pages during the "request for additional documents" phase...
