@robertp I have already sent the request form 5669 to HKVO since they need updated 5669 since age 18 for my parents. But i used 2018-11 version instead of 2019-6 version. Which one we should use? If they need 2019-6 version, what should i do? Please advise. thanks

Eg medical status of your parents?

If so yes. If not then are you asking if you should write an addendum letter or something?

I wouldn't. Why offer information that is unfavorable to your case if they dont ask you for it?

In that case you HAVE to say yes.

If you do not then you are making a false declaration. And intentionally failing to disclose for personal gain.

Of course you could say no and then when found out say " I am deeply sorry......."

i dont think CIC k refuse your application based on medical grounds....There was a famous case that happened a few years ago where they refused an application and parent die after that some law suit was filed and then they changed the policy. So my advise is not to tell CIC any thing for now

Well looks like Canadians are a forgiving bunch. Just say deeply sorry regret my actions. Maybe add in your background and how that has contributed to some "blind spots" that caused you to overlook mentioning.....

How you are going to submit PCC and travel history - scan copy by email or physical copy by postal mail. As if I am not wrong they have asked original Police Clearance certificate
all scan copies..no need to send originals

There will be mis representTion if there was something in the form related to medical disease and you did not declare it. What i am trying to say is that they might not reject the application on medical grounds as they have changed their policy. CIC already knows thats parents and grand parents will have medical issues as most of the people in this category are old.
