I was surprised that the Travel history form is not a part of PR application for so long and it is even difficult to find a copy of this form on Cic website. Finally I was able to get this form and yes IMM 5562(05 - 2004) looks like the latest version available. I am filling this version if you are able to find the newer version please do let me know.
Regarding CV, every information required is part of Schedule 1 Background declaration form I am not sure why they are asking.
I will use this version of IMM 5562
 
@YULIA-2016 I recently got the request for PCC for my parents...I have a question though: I had submitted their PCC in July 2017 with the application package but CIC has again asked for it. I know PCC was not part of the package but I still sent it. Do you think I can use the same PCC? or need to get a new one. How long the PCC is valid for?

I was in same situation like you .i had submitted original pcc along with application in July 2017.on June 4th 2018 officer sent me an email saying that(Documents outstanding are police certificates from All country where your parents lived more than 6 month ).
I replied to the same email by attaching the same old pcc which obtained in 2017,also I mentioned in email I have submitted the original pcc along with application for your reference here by I am attaching the same pcc once again.
On June 7th I received PPR email .
Try uploading the same old pcc.

I am also asked to fill in 'Details of Government Employment' for both my parents. My father in law is retired Govt employee but mother in law is still in service and they were asked to send their Govt employment history again.

They sent one form to fill in the details but it doesn't have any IMM number on it, looks like a simple excel template asking start/end dates, position, title, duties, supervisor name, department and location.
Well, the original application form (IMM5669) itself had those details but I think what they want now is in more details (especially on unit/department and duties).

I think may depend on the age of parent or grandparent. I submitted my 84 year old mum's PCC last year even though it wasn't requested. I asked in an email if they want it renewing and they said not necessary.

It depends on how many people you sponsored. All applicants need one if CIC ask for.
 
Today my ECAS shows Decision Made for Permanent Residence Application Status !!!
But my CIC Online Linked Account still says "Your application is in progress." for Final Decision.
Does this mean they're almost done and I should expect PPR soon?
