Dear Muatiq,

Thanks for your prompt response. I am unable to write on the update information chart even with adobe acrobat professional. I have made another form as per the same format on MS Word. I have then filled it using MS Word and got it printed. Do you think that is fine and non-objectionable for CHC not to fill (by hand or computer) on the original Form.

HAS ANY ONE AT THIS FORUM CREATED AN 'UPDATED INFORMATION CHART' BY USING MS WORD AND SENT TO THEM. I JUST WANT TO KNOW, IF THEY DO NOT GET STINGY WITH SUCH INITIATIVE OF ME.

Regards,

Dear RNSS

Just fill with Black ball pen you will be fine. Do not try something new which might delay your case further.

Good Luck to everyone…
After passing through re-medical examination in December 2013, I received PPR1 6-weeks ago. Now waiting for PPR2. In PPR1 I received email instructions to submit only Passport copies and 2 pages Update Information Chart. I was not asked to submit photographs. I am worried if I missed to submit photographs. Could you please share your experience of PPR1, whether CIO-London asked you to submit photographs or not???
Good Luck to everyone…

passports and chart of information, sometime photos as well.

I think they are accepting only Hard copies and not the soft copies.

If they need photos, it will be mentioned in the letter. However, the stereotype letter does not include the requirement of photos. I was not asked for the photos. So be calm, you might have not missed anything.

It should be page number 3 of your PPR (or PPR-1) document sent by CHC.
