Of course every one has a different number of family members. Some apply alone, others with spouse / kids.
(I did for a family of 5, self, spouse and 3 kids).
We did emedicals. The important thing here is "Non EDE". I have seen worker / visitor / family put for different people who got through irrespective of their status in Canada (I am referring to ppl with CEC who are working in Canada already).
Non ede means excessive demand exempt, which basically excludes the CIC medical condition of putting burden on Canadian healthcare system which is only exempted if you are say a refugee or spouse of a pr or citizen. All economic class applications (CEC, FSW, PNP etc.) should be NON EDE.
Also to be clear that this medical is for permanent residence - express entry, your file that the doctor uploads should have your ITA's copy attached.
There have been cases where medical done was for PR, but the doctors communicated it as for temp residence (work permit). This was for upfront medical exam. So case was refused / reopened and took a long time to figure out.
