Hmm. Unless the rules have changed in the last two months, the medical is still optional to do upfront, but strongly recommended to be included by most. Not doing it up front could and likely will delay you getting PR, since they'll contact you, requesting it be done.
Your medical will expire after one year, unless extended by the Case Officer assigned to your application. The date of your medical is the date that your COPR will be valid until, too. So, if you get your medical in February 2016, submit your application the same month, and get COPR in the mail September, your COPR will be valid until February 2017.
If, for whatever reason, you can't afford a medical up front, you can still send along the application with everything else (including Police Certificates).
This also applies for Police Certificates. Some have sent applications without police certificates and notes, stating that they're ordered but not yet arrived.
General consensus: do both, upfront. It saves you time in the end and leaves less room for error on the CIC's part.
Reality: you
could send your application without a medical and do it later when requested, just as you
could send your application without a police certificate and send it when requested. You naturally run the risk of CIC employees who don't know the rules and send your application back, and the inevitable delay, but you do have the option.
