There's not really an "instant" denial; it will still take a lot of months for the app to be reviewed and then denied

Main reason is that CIC determines the relationship isn't genuine. They do call interviews first to clear up any doubts about the relationship. But if you have a look at the appeals thread, quite a lot of them did have interviews and the officer didn't believe them and denied the visa.
Being denied for a medical reason is incredibly rare, as the excessive demand rule is not applied for spousal sponsorship. I believe they can only deny on medical grounds if the applicant has a highly contagious serious disease that would be a danger to the public. So it's rare
As someone else said, criminal inadmissibility as well.
If the sponsor cannot prove intent to return to Canada, is on welfare or is still subject to a previous sponsorship undertaking, then the app will be refused at stage 1.
