Same form you use it for list of Goods Carrying and goods to follow.
List out Goods Carrying in one from.
List out goods to follow another form.

I carried Bank Issued letter and Account Statement. They didn't even asked me about proof of funds.

What I meant to ask was is there a form for list of goods on cic web or we just list out our own on doc or xls file. Thanks.

Change of address - Inform both AINP and CIC

You need to update both AINP and CIC
Fax No for AINP : (780)427-6560 along with your file number.


For CIC, Fax No: 716-858-9591
Email: buffalo-im-enquiry @ international.gc.ca
You should have received a change of address form along with your AOR. Use this form to fax or to send an email.
Send along with Name, Date of Birth and File Number.

Don't expect any acknowledgement from CIC.

I have exact same case as yours, wrong DOB in medical forms, could you please let me know how you resolved it.
Did you wait for the CIC to send you the updated medical forms and then you went for the medical or you went to medical with the wrong form and simultaneously update the CIC about the discrepancy in your form.
Please let me know

Does CIC send out RPRF and PPR together or separately. For those who completed the medicals during the first week of Dec. 2010, when do you think the RPRF will get sent out?

Nothing so far bro.I have even checked the application status page online and it has not changed .Not even a file transfer.

I have also not been active just due to the sluggish pace at which things are moving. Unfortunately looks like my original estimate might come true. May CIC applicants will probably see a file transfer to LA/Detroit by 1st or 2nd week of March...probably not to New york since Newyork also has slowed down considerably.Also my case has an additional twist , had not submitted FBI with the app.Later on got the FBI certs in June and with the additional delay had again requested fresh FBI certificates and received them in Dec 2010. Now am worried if the process stretches till March/April if they might not consider both FBI certs....and I will be looking at a very tiny window of getting the FBI certs,mailing them before the validity of the medicals runs out.

All this even without background checks figuring into the discussion .

Hard to be patient...but looks like the only option.
