What is the next step?
The next step is to wait, as no one can give you an exact date. Hopefully you hear something by end of the month. My brother has all except background completed as oh Jan 25. Hoping to get passport request by end of the month.
wait! What? I have exact same thing on my account? Tracker shows BGC and medical is complete on Jan 16. Haven't heard anything back yet.
Does it mean my application is approved?
It means that ur application is almost at final stage, if eligibility is also completed that means very soon u will get good news. Jst wait and relax.
Hey my timeline is also exactly like yours, I’m just one day ahead of you and I can clearly see a pattern. Don’t you see the same? And as @Amrish1186 said we will have to wait for another good 10-20 days for the PPR.

In my case after completion of Medical, Biometrics and eligibility it took exact one month to reach final decision. And till that time BGC was in progress. Suddenly everything got completed and I got Passport request. So I will suggest jst wait bcoz when I called them they told if ur file is completed then also it depends upon Visa office when they give final decision, so many things depends on various factor.
What’s the status now ? As my timeline is also closer to yours (just curious)
It really depends on the VO, mine is Calgary. I received P2 the same day but after uploading my photo on Jan 25, no eCOPR yet.
Just got portal 1! 
I just checked and all fields show "completed" on my tracker and there is a COPR date. YAY!
Unfortunately, the people from the Medical test forgot to send my husband's resultsand his medical was entered three days after mine in the system. In his case it appears as Eligibility "not started". I'm the main applicant though, so hopefully his is reviewed soon.

Is it possible for the tracker to update to completed in the middle of the day? It's silly but I feel like when I check it in the morning and there's no update, I lose all hopes for the day. Lol. I called IRCC on Friday and they are saying that my Eligibility is already completed. In the tracker, my Eligibility and Background Verification are still in progress. The wait is really frustrating. 
And promptly managed to mess it up lol.Just got portal 1!
The wording of the message is super unclear but you should NOT create your own account in the PR portal. They will do it for you and send you the logging info on Portal 2.
As I just found a lot of people make this same mistake
and of course most haven't seen this issue until after it happened and you are not aware of this.So here is my word of caution for those waiting for portal 1.
Portal 1 = reply ON THE EMAIL answering their questions (address, name, entry to Canada etc), do not create an account on the portal yourself.
Portal 2 = they provide the login info for the portal, and then you upload pics and whatnot.
Sigh... Creating the account yourself is such a common error that it is apparently not a big deal, but somewhere else on the IRCC web it does say "Do not create your own account. This can delay the process. Follow the instructions in the email we sent and we’ll create it for you."
I wish they included this in the email, but oh well.
I'll let you know how long it takes me to get portal 2 now.
