If you get it done upfront you might save some time. With covid delays, even if your medical expires IRCC will take some time to send you an email, or maybe they will auto-extend the validity for another 6 months. Doing an upfront medical is more of a precaution to avoid further delays, and to get everything up to date from our end.

I am .. AOR dec 2019.. Country of residence India, Stream FSW-O.. As per GCMS my application was approved in Apr 2020.. God knows whats happening and when IRCC plans to provide an update.. My life feels like its put on hold thanks to ircc.. Its ridiculous waiting with no idea what they intend to do.. I keep hoping that I will wake up to something related to a PPR but nothing.. Seems like a distant dream..
‘Anyone’ sounds more like few, I’m thinking ‘everyone’ cuz there are so many people with Nov 2019 AOR, I’m one of them,
my application was approved in March 2020, and I thought IRCC has highest standards of fairness, like first come first serve and all.
I’m wrong, it’s uncouth and appalling way of processing applicants with no proper plan or strategy, which is costing our lives now.
True also. All we can do is wait. Hopefully they will issue CoPRs by September 9 when borders are already open for tourists.

From my knowledge medicals can be extended only once, at least according to the policy used by IRCC in the past. If they expire again they will ask you for new medical exams. I would personally suggest to go for upfront medicals when you are close to the expiration of the extension or to wait for instructions.
