If you are an independent contractor, you are self employed. As per definition an independent contractor is not an employee. You will have to put the city and country where you work.

I had an OPT (optional practical training) application rejected while I was on F1 visa in the US because I did not complete the application by the deadline. Does this fall under any of the statutory questions? Is OPT considered a "permit", because one of the statutory questions involves both visas and permits

It should not. It was a sub status that falls under your F1 and does not get recorded on your passport if I correct.

Yes, an OPT rejection falls under the statutory questions and should be answered accordingly.

Got my Aor on 12th November . I did represent about my USA tourist visa refusal in 2015 however during my canada tourist visa in 2018 i got approved and i wrote a NO in a question asking about prior refusals . would that affect on my Permanent Residence application.

It should not, but you should clarify this in your application that you inadvertently forgot to mention the refusal.
