Hi I submitted application (express entry) they(cic) asking additional document- uci number+ my name?? What is the document meaning??

Could you provide more information regarding which document IRCC requested from you?
Otherwise, it is hard for the forum-mates to advice ...

You should be able to upload completed pdf from your online account.
In your PR application (online account), click on the "View submitted application or upload documents"
Carefully, scroll through the list of transmitted documents and find the link
"Authority to Release Personal Information to a Designated Individual (IMM5475)"
to upload your form.

In the pdf form there are two confusing fields
"Name of the office where the application was submitted"
"Location of office"

Unless, somehow, you know this information explicitly leave these fields blank.

As I mentioned in my previous post, other people are trying to put "something meaningful", like "Online Express Entry System".
IRCC guys are aware that most applicants have no clue of where their case being processed.
And they obviously don't want thousands of people contacting them to figure out this information.
Otherwise, it would be easily available for you in your online account.

Thanks for the thorough reply! :) I'm sure they don't need loads of people asking them where their case is being processed. I wouldn't care either except that in order to raise a Webform this info seems to be necessary! Anyway thanks for this, I'll take a look and see. Much appreciated. :)
