What should be the answer for statutory question :have you ever been applied to ircc before??
If we are already having work permit.
Do we have to say yes or no for this question??

You should answer yes. I applied for a visit visa and got rejected the first time. I had to mention that with an explanation as well.
 
They note it in the file only if your ages changes between ITA and AOR. My birthday came after my AOR and both the notes I ordered did not decrease the score.
Ummm.. well, my birthday was after AOR and ordered my notes before my birthday. I saw the next recalculation for eligibility was the same day of my birthday. I haven't ordered notes after my birthday to see if they actually lower my points though.

Eligibility on progress, backround check on progress, criminality and medicals passed..
I have this status since january..I dont understand.
I am ordering GCMS notes now.

No update since then, when I called up ircc yermsterday, she said she does not have the file with her and cannot provide me any information that I have cleared the medicals. I remember that file had been sent to somewhere else .

You will have to decide that - it doesn't make any difference. The letter should be written keeping the NOC reference provided by IRCC in mind. Follow the IRCC recommendation and keep it simple. Ideally, the person who wrote it should be the one signing it - because the person is familiar with your work, the content and can attest to it should there be future verifications. If it's the manager who wrote it considering he knows your responsibilities and HR can confirm and sign on letterhead, that's ok.

Did it specially show a decrease in points? They generally note it specifically mentioning the new CRS score. I specifically checked that in my notes cause I was worried about it.

I ordered in March and in April however nothing was highlighted and my birthday was in December.

I haven't ordered a new set of notes after my birthday so I am not sure if they actually decrease my points but as I mentioned before I saw the next recalculation date on the notes ordered before my birthday. My points are over the cut-off score so I didn't bother ordering new notes and also I feel PPR is around the corner :):)hopefully..

That is what I am trying to tell these guys and they are debating this with me, its also on my notes and I also called and they explained to me so I don’t know why they can’t just accept things from someone who have experienced it first hand

They took 5 points from my score I have it in my notes just like I was explaining to them previously, they need to understand that a decision has not yet been made so if you have another birthday before they get to your file they will remove 5 points
If the 5 points taken makes your score lower than the lowest cut off score then they can’t refuse you because of this and have to accept your initial score in this case
