ako december 28 applicant.. nag PPR ako june 25. any news sa mga november applicant dyn?..

PPR na ako kahapon lang, will send my passport today...DHL ba ok lang? or LBC?

Do i need to submit my AOM or appendex A lang?

Only submit the documents that are specifically stated in the letter/e-mail.

How bout you, did the CEM ask for the AOM, kasi sabi ng iba, isabay ko nalang daw yung AOM kasi based on their experience kahit passport lang hiningi at first, after 2 weeks hiningi yung AOM...

Do i need to send it both nalang, is ok ba? Appendix A, passport & AOM?

3.) copy of the email (stated at the bot of the email to include a copy)
4.) aom (optional .. Not requested but as you can see all the applicants had been required to give one.. Why wait for a next additional doc request if you can just send it now)

Hi, tiffanyeric... Wala naman ibang hiningi pa sa husband ko. It's been over 2 weeks now since they received his passport.
Btw, either dhl or lbc is fine to use. Both my mom and my husband used lbc in sending their passports.

3. Spouse: Is it me or hubby?
4. Original Signature of Principal Applicant: My signature?

Uhhh.. Applicant is the one in PH.. Spouse told my wife to left it blank since im already here..
I didnt see any box asking for uci number in appendix a ..

Spouse would be wife or husbund of applicant
Signiture of the applicant the one who is in ph

Eye color

Prpbably just any valid id.. I had my wife's aom taken by someone.. She only asked us to provide an authorization letter to get one and a valid id .. Im not sure how to get it personally

All the required info is about you, being the applicant. Don't write anything on the spouse column as he is your sponsor, not your dependent. The signature would be yours.

What!!! Pwede mo ba kuhaanng picture at post yung appendix a mo?? Mine didnt had any uci or application number blank on appdenix a
Yup yup spouse mo sponsor so leave it blank ..

My wife's appendix a only required her to fill up the 1st column for the applicant.. Then ask for contact number mail address and signiture

Hi,As far as I remember meron yung File no# sa itaas yun yung number na may F000----- yan ang ilagay mo sa File no# Tapos yung sa applicant ikaw mismo ang magfill up dun at dun naman sa spouse yung sponsor mo ifill up mo at leave it blank dun sa passport # kasi ikaw lang ang dapat maglagay nang passport details mo.Dun naman sa signature ikaw ang magsign dun kasi ikaw ang applicant and dont forget to put a date after mo magsign dun sa signature .Child 1 ,Child 2, Child 3 leave it blank kung wala kang kids.
Yun naman sa Advisory on Marriages you can get it in one day sa main office nang mga NSO. Just fill up the Cenomar form at lalabas yung Aom mo if you are married .Just bring one valid ID and Payment then you are ready to go Good luck!

Then APPENDIX A:

File #:____________ was asked...so what will i put, my applicant #?
