Do i have to include my notice of re-assessment aside from my notice of assessment same year 2011?

Thanks red and black! I noticed there are 3 pages of assessment. Do i have to include all or only the third page which has the summary of assesment?

I'd include all of them. Just organize it in such a way that will allow the visa officer to distinguish between the assessment and re-assessment easily.

Not relevant to the topic but just want to ask: are only family and given names to be written in full name under personal details? How about the middle name?

I dont think you need to provide notice of assesment.

Because it is final assesment which matters.
Moreover there is no information which assesment has and re-assesment does not have.

Re-assesment is actualy your real assesment.
