I have PhD (Chemistry) and post-doc from US, and was teaching assistant for 7 years while doing my PhD.
I do not know whether to pick 4121 or 4131? I also see 4122 (TA, post doc), but it is not in 38 eligible list, it appears in only secondary title list?

I meant from your post-doc professor. He will be the one who will write the current employment letter. If you do not have a cooperative post-doc supervisor, try to see if you can get such a letter from your HR person. HR people from the department are usually very helpful and accomodative. I have better relations with my PhD advisor than my post-doc professor. Will it help if I get letter from the former?
