Could any one explain me about the PLAR in above... Do I have to do the course and attend the exam... or just to present a notarised affadavit like '' context of physiotherapy practice in canada'' to prove that I read and understand the topic.

I am assuming this is your first evaluation result. In that case, you have 2 options at this stage:
i) You may chose to request the Alliance to do a supplementary review if you believe you covered these areas during your studies. For this you will need your college to provide written clarifications in relation to the identified areas. They (your school) do not have to amend or re-write your course description. They only have to provide a clarification and/or further details on each identified areas in the form of an appendix to the original course description. This is what I did in my case and it was accepted my the Alliance.
ii) If you however agree with the outcome of the current evaluation as it is, the Alliance will provide you with the available options for you to complete the applicable modules (institutions, online courses, contact details etc). They will however not provide you with this until you have applied for PLAR.
If you can, I'll advise you explore the first option as it will save you time energy and money. I am not sure where you are currently based, but sometime, it could be difficult to take the available options for some modules if you are not currently in Canada.
