Guys, i started to collect documents for SVR,as i read and understand that these documents r required for sending to MCC,please correct me if it is not:
1-copy of my passport( since every detail in passport is written in english,french and arabic,so should i certify this copy OR just do copy and send it??)
ANS: People always have their passport written in their native language and the second common language used. Since your passport is written in English and french( 2 official languages in canada) its fine.
2-i should send passport: cover sheet, right??
Ans: Passport cover-sheet is required for them to identify that the passport copy is indeed a passport . Passport copy must be notarized. Please look at the specifications on how to notorize the passport photocopy.
3-Certified Identity Confirmation also
Ans ( It must be filled by the notary public in your presence,duly signed and stamped by him.Remember the photograph on top must be either signed or stamped by the notary public on left or right upper part of the photograph. The bottom photograph must not be signed or stamped. This photograph will be used by them in your physiciansapply.ca account
4-copy of my Medical diploma with original Translation,right?? and 2 or 1 copy have to send ?
Ans: If the degree/diploma is in other language other then English or french then it must be translated. Please send them the copy of original diploma and the original translation. Use the cover sheet as it will help them to identify that the document that you have sent is the copy of your diploma.
5-Medical degree or diploma: Cover sheet Print and send it also??
Ans: Yes medical degree or diploma are the same thing .Print the cover and use it
6- 2 photos,first to attach firmly with Identification Confirmation and the second not attach it firmly.
Ans: second photograph will be used in your physiciansapply.ca account. so attach loosely.
7-Release form Print and send it also,right?
Ans : yes mention the date and just sign it.
8- my studies start date is 2001-09-10 and studies end date is about 2007-06-10, so will they calculate and count it ,in my diploma is written 6 years but if your count period between these 2 days,u will notice less 6 years,will it be problem??
Ans : End date is generally the date when you take your final exams / or when your results are published, that is when your studies officially end.
They will use your diploma and make sure that your medical school is listed in the IMED directory. If it is listed then no problem, then they will send it to ECFMC for source verification.Since your degree is already SV . It will save your time.I have noticed a change in the physiciansapply.ca account and new users may not have to mention their commencing and ending dates of their studies ( please confirm)
The other thing i have noticed at the MCC website is that they clearly state that ECA request made prior to december 2014 will be honoured. This is a confusing status where we are not sure if they will process ECA of candidates applying after december 2014. But please send them a message from your account and confirm.
9- last question ,please,who will complete Certified Identity Confirmation?? since i am abroad of my country now,so sworn turkish translater will be ok to certify that??
Ans : A notary public wherever you are unless stated otherwise.
Many questions but Guys u know i don't want to make any mistake although as i said before that i have already ceritified my education crednetials documents by ECFMG coz i applied for step 2 ck and i will send a document with my usmle number clarifying that to MCC, so please i am waiting your help and reply as soon as possible,really appreciated ur help.

Ans: First open the account with physiciansapply.ca . from your account send them a message stating your EICS number and Your USMLE number. within a week they will update it to your account and when they send your document for SV they will attach these numbers on your diploma certificate. Since your degree is already source verified it will save your time ( nearly 100 days)
