I think we have to pay 2 more fees one is ECA report fee 98cad another one is courier fee 134 cad

No need to write name on the back side dear. Certifying official will match your photo with your other Identifying Documents (Passport or ID card) as well as Physically and verify your Identity.

i sent all my documents through DHL and also paid the Document and ECA fee....Whats next vhia?Is there any other procedure we have to do through Physician.ca?Any other charge? i am not sending any documents which are differ from other FSWP 2014 applicants
pl check:
i opened an account with paying 260CAD
then i paid document fee (for MBBS certificate) 145 CAD
and am going to send these documents:
1.Certified Identity confirmation page
with Notarize passport page also Passport Cover sheet
2.Medical Degree cover sheet with Notarize MBBS Certi.
3.Consent Release Form

am i missing anything ???

no, just print ur Personal details page with ur picture page as well and just notarize it according to sample.
