i think if u send Intern certificate they will charge for it although for the FSWP u dont need.

In the ECFMG website it has stated that "To be eligible for ECFMG Certification, international medical graduates must provide copies of their medical education credentials—currently the final medical diploma, final medical school transcript, and, if required, transfer credit transcript(s)."

In my list contains also six points. First five are the same as yours. Just wondering...the last item 'Declaration form' is that same as 'Release form'?
Yes, We need to certify the degree certificate as well.

The following documents will be required:
1. MBBS Certificate with Transcripts
2. Your Passport's copy
3. IDENTITY CONFIRMATION form sent by MCC
4. CONSENT form sent by MCC

All of the documents must be certified by Authorized body.

Hello Arshi

Once you create an account on MCC and then go for Source Verification (SV) request. There you need to fill up your passport information and your medical degree information after filling up these information, you will get the option/link to download cover sheets in pdf format. After submitting SV request you will get also the link to download the CONSENT / Release / Declaration form (this is a single form whatever you named it).

Dear,

MCC will send your documents to Educational Commission for Foreign Medical Graduates International Credentials Services (“ECFMG/EICS”) for the purpose of verification and/or source verification and MCC needs to share your information to ECFMG/EICS. For that reason you have to give authorization to them. And, this is the "Consent of Release".

The required documents are mentioned in the above posts. You need to attest all the documents before sending.
