Leave blank of these fields. And, I don't think these cover pages need to send for Verification. I didn't send them.

Since they are asking to print it and send it as a cover page, so i would recommend just send as it is. No need to fill any extra info.

I am going to do the same. Let's see how long the ECA report takes for MCC.
I think those who already submitted and get the ECA, it would be really nice if they can post their timeline and which country from.

is it ok.......or u are sending something else along also.......one question is that do we need to get degree copy certified or no

In my list contains also six points. First five are the same as yours. Just wondering...the last item 'Declaration form' is that same as 'Release form'?
Yes, We need to certify the degree certificate as well.

i guess , it will be done depending on your duties and of course the letter of experience you get from the employer as MCC clearly stated to me they ONLY ASSESS primary degree for all doctors

I think you can get International Debit/Credit Card from Standard Chartered Bank (in Bangladesh) or you can take help from someone who has Int'l Card.
