* Payment of the Application for Permanent Residence Fee for your new dependent if he is accompanying.
* Copy of State issued birth certificate for your new dependant. Please note that hospital birth certificates will not be accepted
* 4 passport-sized photos for your new dependant, as per the photo specifications found in Appendix C of the visa office specific instructions.
* Copy of the biographical page from valid passport for your new dependant.
Please be advised to send the above documents within 60 days of the receipt of this e-mail.

Yes I have done my medicals and you can just walk in. Asaju opens 9am for business so will have to go early. 28k for adults kids up to 3yrs 20k and 6yrs-above 22k.

Yes. Did my medicals on Monday at Asaju clinic.
Adults = N28K each
Kids = N22K each (5 and 7 years of age)

I paid cash. When I called to book the appointment, they confirmed that they collect cash. I didn't ask of the card option.

I had an addition to my family early this year so I filled and submitted those forms when I contacted AVO to let them know of the addition. I also sent an updated POF to reflect the new family size then. So I wasnt requested to submit either form again.
