2. Schedule A
3. Additional Family Information Form (5406E)
4. Notarized copy of the Bio page of baby's international passport
5. Notarized copy of birth certificate
6. Covering letter of what change took place in your family composition
7. Updated proof of fund to cover the child ( This is debatable. A lady from Ankara VO submitted all of the above docs without the POF) and she had her Ugwu leaf already
