its does not matter...my passport bears my maiden name....even after i sent my change of name documents to accra...my medicals came in my maiden name.....the work with the name on the passport

Maky, I believe your maiden name was what you put on the initial application and VO just continued to use it even after you legally changed it. I am not quite getting Diamond x's question.

My wife madiens name is still on her passport bt the medicals came in my own surname. The hospital says we can use it n support it with marriage certificate.

What I have observed with this Canadian immigration process is that those who have advanced in the process worry unnecessarily; they always find something to worry about...

I don't think u have anything to worry about. In ur initial applic package, i beleive u must have included a copy of d passport u're refering to. If it was not acceptable to d vo they would have said it. Also there is a part of ur appl where ur wife's maiden name, infact, parents' full names were requested. So i think all those are sufficient to establish ur wife's identity.
However, if u are still not ok with this, go to alagbon and ask observation of ur name to be made on her passpt.

I am not sure notations are allowed on e-passports, so going to slag on is out of the question. I believe he should just request that his lawyer send him a copy of the forms sent to CIC. Also, it's funny that he didn't see the forms bc if I recall correctly, an applicant needs to sign the application form.

I will advice you request for a copy of your original application form, if it is your wife's maiden name that is written in there then o problems but if the forms are filled in her marital name, I will suggest you draw accra's attention to it. One thing you need to know is that identity theft is real abroad and you need to ensure that all documents pertaining to your and/or your family's identity is properly captured from day one.
