Oh youre the second person to ask about that consent form. Those are the documents I submitted also, except that consent form. I just gave everything they asked from the PPR email, unless they changed something since April 7th. I will pm you the info that VFS sent me regarding the passport submission when I asked about it.

---XABIBELLA - ask ko lang sa vfs website, under nung "how to apply", dun sa "submit your passport" lang ang dapat tignan dba? hindi na yung "submit your application online" & the others aside from "submit your passport"... dun kc merong consent consent eh... is it ideal to just go in-person or by mail?
thaks in advance...

Natanong ko na to sa vfs if personal no need consent pero if mail need ng consent. Antayin mo na lng din response ni xabibella. 1 page lng nmn yon yong second page no need na ifill out if ang mgpapadala and rerecieve nmn yong inisponsor. Better personal kasi cash payment don pag mail ang alam ko nka managers check

Thank you for doing that. :) When my husband is going to have his Medical, does he needs any kind of papers that he needs to pass to the Doctor's office? And what is the best time that he will get his Medical done? is it when we're about to finish our package? isn't it it is included on the first package that I am going to pass here in Canada?

One more question, I am doing the Financial Evaluation IMM1283, and there's a question, "is your spouse or common-law partner co-signing the undertaking?"
...and how did you filled this up? I am so confused with the succeeding questions about the undertakings... :( I don't know what to put/write.

At the Generic Application Form For Canada,

I come across at the bottom of the questionnaire intended for Dependents, can we just remove this part?
NOTE: We don't have any dependents.

Thank you so much!
- For the doctor's office, depends on where your husband will go so call them ahead of time. (For mine, I just had to bring my passport and I filled up a form when I got to the doctor's office). I had my medical exam 10 days before we submitted our application because I wanted to avoid the re-medical request. And yes, you have to include it in your package. Also, you said "first" package, just a reminder that you only submit the application once all together with the sponsorship and PR application in one package to Mississauga.

- You don't need IMM1283. That form is only if you have a dependent child http://www.cic.gc.ca/english/information/applications/fineval.asp What you need is IMM1344 and IMM5481E from here http://www.cic.gc.ca/english/information/applications/fc.asp I had to look at my forms again to remember what I put: http://i.imgur.com/3iWK8vC.jpg
** AGAIN THIS IS IF YOU DON'T HAVE DEPENDENTS/CHILDREN/ETC.

- and yes, you can remove that part for the dependents in the Generic Application form :)

hi shemah! yayyy congrats on the PPR! yes, you only need the stuff from "submit your passport" the other ones are if you are applying for TRVs and such. I would suggest go in person so if they have questions or something is missing in the requirements you'll know right away. Also you avoid the risk of your passport getting lost in the mail!

hello po sa mga PPR na tanong ko lang po about sa Appendix B form need ba isulat yung information ng spouse/partner at kung canadian citizen na yung child na may hawak ng passport? salamat po sa sasagot..
As far as I know, you have to fill up the complete info for everyone in your family. I filled up the info for my husband (including passport #) and he's a Canadian citizen.
