ive asked the same question before, share ko lng what i learned. kapag nagrequest ka sa NSO, CENOMAR ang tawag. if your single, form#4 ang ibibigay syo, if married ka na form#5, un ung AOM.

when you request sa NSO same lng ung request form for Cenomar/AOM. When your single ypu get CENOMAR, when yout married you get ADVISORY OF MARRIAGE. .

my hubby requested for the cenomar online & paid through the bank, we received an email saying it was out in the mail & we'll be receiving it within 10 days. or ur wife could also go to NSO.

AOM lists the marriage(s) of a person, i think that's the reason they ask for it. they wanted to see the applicant's marriage history. NOT SURE about this though!

She can request by going to NSO and get it immediately or depends per area ( i read here in manila it can take up to 1 week) at about 195php

but if you opt for the ecensus it will cost you more but they will send it by mail to your home address so less hassle on her part. NsO offices tend to have a lot of people so she'll.wait in line for sure.

If your not in a hurry and dont mind spending some more, get it via ecensus.

Thanks toffboss. I requested via ecensus but after paying they said it will take 6-8weeks. At the same time my wife SMS'd me and said she'll also go NSO today to request for a copy. Once she get's it she'll FedEx it to me. So unahan na lang sila.

Nakalimutan niu po ba isama sa apllication niu? Pwedeng d na nia ipadala sayo. Manila Visa Office will add it sa request nila when they send an email requesting Passport and Appendix A.

oo nga, it's better if ipasa mo na at least umaandar na application nyo. hindi din kmi nakapagpasa, maghihintay na lang kaming irequest ng CEM.

So I just submitted Appendix A and our Passports. Now I'm worried about the attached Appendix B. Do I need to submit it right away or wait till they ask me to submit it.
